Streptomycin Sulfate Dosage and Administration

1. Usual Adult Dosage
(1) Intramuscular injection, 0.5 g (calculated as streptomycin, the same below) once every 12 hours, used in combination with other antibacterial drugs. For bacterial (Streptococcus aureus) endocarditis, intramuscular injection, 1 g every 12 hours, used in combination with penicillin, for 1 week, followed by 0.5 g every 12 hours for 1 week. Patients over 60 years old should reduce the dose to 0.5 g every 12 hours for 2 weeks.
(2) Enterococcal endocarditis, intramuscular injection, used in combination with penicillin, 1 g every 12 hours for 2 weeks, followed by 0.5 g every 12 hours for 4 weeks.
(3) Plague, intramuscular injection, 0.5-1 g once every 12 hours, used in combination with tetracycline, for a 10-day course of treatment.
(4) Tularemia, intramuscular injection, 0.5-1 g every 12 hours for 7-14 days.

(5) For tuberculosis, administer intramuscularly 0.5g every 12 hours, or 0.75g once daily, in combination with other anti-tuberculosis drugs. If intermittent therapy is used, administer 1g 2-3 times per week. For elderly patients, administer 0.5-0.75g once daily.
(6) For brucellosis, administer 1-2g daily, divided into two intramuscular injections, in combination with tetracycline, for a course of 3 weeks or longer.

2. Pediatric intramuscular dose.

3. Patients with impaired renal function: The normal dose of streptomycin for patients with normal renal function is 15mg/kg intramuscularly once daily. If the creatinine clearance rate is >50-90ml/min, give 50% of the normal dose every 24 hours; if the creatinine clearance rate is 10-50ml/min, give 50% of the normal dose every 24-72 hours; if the creatinine clearance rate is <10ml/min, give 50% of the normal dose every 72-96 hours.
